Essentials - $99 a month. Includes:

• Up to 2 office visits per year with one of our experienced advanced practice providers (e.g. nurse practitioner - NP, physician's associate - PA).
• Guaranteed same day visits when requested (priority scheduling).
• Business hour access to your provider via email, text, and portal messaging.
• Prescription management.
• Complimentary preauthorizations, records requests, scheduling of testing.
• Complimentary hormone panel (e.g. testosterone, estrogen).
• Complimentary trial of GLP-1 agonist for weight loss (e.g. semaglutide)
• 20% off discount for additional visits, procedures, and supplements.

Best for patients who require periodic visits for chronic urological conditions on medications (e.g. bladder/prostate issues, recurrent UTIs, kidney stones, ED, prostate cancer screening).

Select - $149 a month. Includes:

• Everything in Essentials Plan plus . . .
• One additional telemedicine visit per year and an annual comprehensive in-person wellness check with one of our experienced advanced practice providers.
• Free quarterly blood draws.
• Monthly high-quality, medical grade supplement.
• 25% off discount for additional visits, procedures, and supplements.

Best for patients who require recurrent lab testing and more frequent visits, such as patients on hormone replacement/optimization.

Premier - $199 a month. Includes:

Everything in the Select Plan plus...

• Visits with Dr. Joseph Pazona, Dr. Howard Aubert, or Brooke Faught, DNP.
• Limited slots available.

One-time enrollment fee of $199 for all new patients. Fee will be waived for all existing Pazona MD patients who enroll by January 31, 2025. A la carte office visit fees can be applied to the enrollment fee.
